We have had an overwhelming response to this year's Heart & Eye Clinic! At this time, all available time slots have been filled, and registrations have closed.
For those who are signed up, you will receive an email soon with further details, including the location and your scheduled appointment time.
You can download the OFA Cardiac Form with Dr. Gordon's pre-filled information. Please bring the completed form with you to the Heart Clinic. For Eye Exams, a form will be provided to you the day of the Clinic.
Cash payments are preferred. However, we also accept all major credit & debit cards.
